INSTRUCTIONS: For the Department of Computer Science employee’s convenience, change of address forms are located in Room 4172, on the right hand side as you enter the office in a hanging rack. The forms are divided into separate types of employees Faculty/Staff, Hourly, Graduate Research Assistant, Graduate Teaching Assistant and Fellowship.

Use black ink only, print legibility or type your form.
It takes 6 to 8 weeks for this change to be effective.
