Take a look at the templates, then come back here, left-click on the link and use “save target as” to save the file on your local drive.

The forms are web pages which were created in MS WORD and therefore can be easily edited that way. Of course you can recreate the simple look with any other tool…  Just use the same layout/fonts etc., as it will facilitate comparisons.
