Patient Information



The physician must enter in patient information. This includes the patient’s first and last name, weight, height, address, birth date, and a notes section. The notes will be used to state the patient’s rehabilitation issues and recommended solutions. It will also include any updates notes on the patient’s rehabilitation program.

Workout Recommendation


This form provides a list of all possible workouts pertaining to the patient’s specific rehabilitation program. On the left portion of the form, the physician is able to type in a search keyword that will highlight a corresponding workout type. This allows the physician to quickly access specific workouts. By clicking on the arrow pointing right, the highlighted workout is added to the list of recommended workouts used with the Diamond Suit and deleted from the list of all possible workouts. The physician is able to add as many workouts as necessary. On the right portion of the form, there exists a list of recommended workouts for the patient to do with the Diamond Suit. The physician is able to search on this list by keywords as well. By clicking on the arrow pointing left, the highlighted workout is deleted from the recommended workouts and added back into the list of all possible workouts. The physician may submit the request by clicking on the Complete Button or clear all the recommended workouts by clicking on the Clear Button.
